- To reserve a time and date, the Wedding reservation form (inquire for details), must be faxed to the hotel directly. This must be received a minimum of 1 week prior to travel dates to Puerto Plata. A return phone, fax number and e-mail address is necessary to which the confirmation is to be sent.
